1. Contest is open to U.S. Residents 13 years of age or older, except for the
members and their immediate families of Sound Slave Productions (SSPs) and each
of SSPs affiliated companies, licensees, advertising and promotional agencies
and production companies relating to this contest. Any contestant under the age
of 18 must obtain consent from their parent or legal guardian before completing
an entry.
2. You may enter the Halo 5 CD Give-Away by fully completing the official online
entry form available at https://soundslaveproduction.tripod.com/halocontest.htm
and submitting the same. You may also enter by hand printing "Halo 5 CD
Give-Away", your full name, address (city, state, and zip code), and
daytime phone number (with area code), age and if you have one, an email
address, on a 3" x 5" card and mailing it, in a postage-prepaid
envelope hand addressed to Halo 5 CD Give-Away, 1113 Grovemont Dr., Suite G1,
Greenville, NC 27834.
3. All entries must be transmitted or postmarked between 12:00 a.m. Eastern
Standard Time on JULY 6, 2000 and 5:00 p.m. Eastern Standard Time on November 28,
2000. Limit one entry per person. Sound Slave Productions and Halo 5 are
not responsible for any software, hardware, telephone line, U.S. Mail or human
errors.
4. On DECEMBER 1, 2000 Halo 5 will randomly select winning entries from all
timely received qualifying entries.
5. The following prizes will be awarded: one (1) mp3.com CD
"Ascension" (VALUE: $6/US).
6. All odds of winning depend upon the number of qualifying entries received.
7. Prize Winners will be notified by electronic or U.S. mail. Winners (or their
parent or legal guardian) may be required to execute an affidavit of eligibility
and liability/publicity release which must be returned or postmarked within 15
days of the date the notification is mailed to the potential winners. Failure of
a potential winner to execute and return an affidavit of eligibility and/or a
liability/publicity release, or the return of the notification or prize as
undeliverable, will result in disqualification and selection of an alternate
winner. If a prize is won by a minor, the prize will be awarded to a parent or
legal guardian on the minor's behalf. No substitution or transfer of prizes or
cash except by Sponsor due to unavailability. All winners agree that Sound Slave
Productions and their affiliated companies, distributors, agencies and employees
shall not be liable for injury, loss, or damage of any kind resulting from
participating in this contest or from the acceptance or use of the awarded
prizes. All winner's are solely responsible for any and all taxes or other fees
on all prizes.
8. By entering the sweepstakes, each participant grants Sound Slave Productions
and Halo 5 the permission to share the information submitted by participant with
recording artists, music labels, music distributors and others carefully
selected by Sound Slave Productions.
9. Each contestant accepts and agrees to be bound by these rules and the
decision of the judges which shall be final and binding and not appealable.
10. All Federal and State laws apply.
11. Void where prohibited or restricted by law.
12. All entries submitted become the property of Sound Slave Productions.
